Petitioning for record sealing of DUI

Location: Arizona Possibly a silly question, but want to get all the information right the first time. I am petitioning the Arizona Courts to seal the record of my DUI arrest and conviction. The arrest was over seven years ago in May of 2019. When searching my case records, the counts that were dismissed also appear. On the form under CASE RECORD INFORMATION, should I list all 6 counts, or just the one count I plead guilty to? For reference: https://www.azcourts.gov/selfservicecenter/Criminal-Law/Sealing-records
