Writing a written notice to a landlord

Location: NE Ohio Hi all, I have a question in regards to writing a formal notice to my landlord. My wife, 2 kids and I live in a small duplex. We recently discovered a few roaches appearing throughout the day in our apartment. Our landlord, we presume, knew of this prior to is writing this and potentially even moving in, as when another tenant moved in, he proceeded to send the tenant a pack of roach bombs and told them to give us a few. From our understanding, roach bombs are ineffective and with one of our kids being crawling age, we dont want chemicals like that sprayed around our house. I texted our landlord politely asking if he would be willing to hire an exterminator to ensure the issue is fully resolved and I have yet to hear a reply. I want to know what writing the notice would be like, as my wife and I feel disgusted knowing there is a roach infestation and want it to either be dealt with or offered a way to break our lease without any severe repercussions. If anyone has any advice, ideas, or otherwise, it would be greatly appreciated. Thank you.
