42.051. State agency forms to include questions for veterans — information on agency's veteran services to be provided, when — report. — 1. Every state agency shall include on any form, including digital forms posted on an internet website, used to interact with members of the public the following questions in substantially similar form:

2. Every state agency shall provide the contact information of those individuals who answer question (3) in subsection 1 of this section in the affirmative to the commission within seven business days of receipt and shall provide the contact information in a format readily accessible by the commission.

3. Every state agency shall prepare information regarding the agency's applicable services and benefits that are available to veterans and provide such information to those individuals who answer questions (1) and (2) provided in subsection 1 of this section in the affirmative and to the commission. Such information shall be available in a format readily accessible and identifiable to members of the public and to the commission.

4. On January first of every year, the commission shall post a report on the commission's website that includes:

(1) The total number of individuals whose contact information has been submitted to the commission from each state agency pursuant to subsection 2 of this section; and

(2) The total number of individuals contacted by the commission as a result of the submission of contact information by a state agency.

5. The provisions of subsection 1 of this section shall only apply to any form first created on or after August 28, 2024, or any form created before August 28, 2024, and subsequently modified on or after August 28, 2024.
