Distinctive plates for New York state licensed dentists who are members of the New York State Dental Association

* § 404-r. Distinctive plates for New York state licensed dentists who\nare members of the New York State Dental Association. 1. Any New York\nstate licensed dentist residing in this state who is a member of the New\nYork State Dental Association as established pursuant to chapter six\nhundred eighty-three of the laws of nineteen hundred ninety-seven shall,\nupon request, be issued a license plate formatted at the discretion of\nthe department bearing the letters "DDS" or "DMD" and the official logo\nof the New York State Dental Association. Such license plate need not\ncontain the statue of liberty logo. If a distinctive plate is issued to\na New York state licensed dentist pursuant to this section, such\ndistinctive plate shall not be issued to the spouse of such member.\nApplication for said license shall be filed with the commissioner in\nsuch form and detail as the commissioner shall prescribe, and shall be\nverified annually by the applicant.\n 2. The distinctive plate authorized in subdivision one of this section\nshall be issued upon proof, satisfactory to the commissioner, that the\napplicant is presently a dentist licensed to practice in the state of\nNew York and a member of the New York State Dental Association.\n 3. A distinctive plate issued pursuant to this section shall be issued\nin the same manner as other number plates upon payment of the regular\nregistration fee prescribed by section four hundred one of this article\nand an additional annual service charge of thirty dollars. Twenty\ndollars from each thirty dollars received as annual service charges\nunder this section shall be deposited to a fund for the credit of the\nNew York State Dental Foundation, said fund established as a revolving\nfund pursuant to section ninety-seven-www of the state finance law;\nprovided, however, that one year after the effective date of this\nsection, funds in the amount of five thousand dollars, or so much\nthereof as may be available shall be allocated from such fund to the\ndepartment to offset costs associated with the production of such\nlicense plates.\n * NB There are 2 § 404-r's\n
