Ohio Rev. Code § 121.375

October 16, 2009 House Bill 1 - 128th General Assembly (A) As used in this section: "At-risk individual" means an individual at great risk of not being able to access available health and social services due to barriers such as poverty, inadequate transportation, culture, and priorities of basic survival. "Care coordination agency" means a person or government entity that assists at-risk individuals access available health and social services the at-risk individuals need. (B) A care coordination agency may provide the following information to the Ohio family and children first cabinet council: (1) The types of individuals the agency identifies as being at-risk individuals; (2) The total per-individual cost to the agency for care coordination services provided to at-risk individuals; (3) The administrative cost per individual for care coordination services provided to at-risk individuals; (4) The specific work products the agency purchased to provide care coordination services to at-risk individuals; (5) The strategies the agency uses to help at-risk individuals access available health and social services; (6) The agency's success in helping at-risk individuals access available health and social services; (7) The mechanisms the agency uses to identify and eliminate duplicate care coordination services. (C) The Ohio family and children first cabinet council may do either or both of the following: (1) Give incentives to encourage care coordination agencies to provide information to the council under this section; (2) Use the information provided to it under this section to help improve care coordination for at-risk individuals throughout the state.
