Statement of Professional Disclosure - Furnishing to

Official statutory text

client.

All licensed behavioral practitioners, except those employed by

federal, state, or local governmental agencies, shall, prior to the

performance of service, furnish the client with a copy of the

Statement of Professional Disclosure as promulgated by rule of the

State Board of Behavioral Health Licensure. A current copy of the

document shall be on file with the Board at all times.
