Notice of practice location and address - Proof of

licensure.
Each person holding a license authorizing the practice of
medicine and surgery in this state shall notify the State Board of
Medical Licensure and Supervision, in writing, of such licensee’s
current practice location and mailing address. Each licensee shall
carry on his or her person at all times while engaged in such
practice of medicine and surgery official verification of valid and
effective licensure as may be issued by the Board.
