Disclosure statement; providing percentage ranges upon request.

Sec. 12.
Each organization or professional fund-raiser shall prepare a disclosure statement to be given with all printed material and read when contact is made by telephone, to each person from whom a contribution is solicited. The disclosure statement shall contain all of the following information:
The name and purpose of the organization.
Whether the solicitor is a separate organization acting on behalf of a public safety organization.
The specific purpose or purposes, including any political purposes and campaign contributions, for which the contributions are to be used.
That the categories and percentages of distributions of contributions are available upon request as provided in subsection (2).
Upon written or verbal request of the individual being solicited, the solicitor shall provide the percentage ranges for each category as described in section 3.
