§ 33-0703. Application for registration.\n The applicant shall file with the commissioner a statement including:\n 1. The name and address of the applicant and the name and address of\nthe person whose name will appear on the label, if other than the\napplicant;\n 2. The name of the pesticide;\n 3. A complete copy of the labeling accompanying the pesticide and a\nstatement of all claims to be made for it including directions for use;\n 4. If requested by the commissioner a full description of the tests\nmade and their results upon which the claims are based. In the case of\nregistration of a pesticide which has been registered for the previous\nyear, a statement shall be required only with respect to information\nwhich is different from that furnished when the pesticide was last\nregistered; and\n 5. For restricted use pesticides, if requested by the commissioner a\nSpanish or other language translation of the label including directions\nfor use.\n
