N.Y. LFN Law § 141.00

§ 141.00 State comptroller to prescribe form of debt statement. a.\nThe state comptroller shall prescribe the form of debt statements and\nmay require that there be submitted with such statements such other\ninformation as he deems appropriate.\n b. The state comptroller shall prepare printed forms of debt\nstatements and shall make them available for distribution to\nmunicipalities, fire districts and school districts.\n c. Notwithstanding the provisions of any other law, the state\ncomptroller shall furnish to any person upon request a copy of any debt\nstatement filed in his office pursuant to the provisions of this\nchapter, upon prepayment of the following fees:\n 1. For a copy of any such statement, one dollar and fifty cents;\n 2. For a certified copy of any such statement certified under the seal\nof his office, two dollars;\n 3. For an exemplified copy of any such statement, three dollars;\n 4. For a copy, a certified copy or an exemplified copy of any debt\nstatement, an additional fee shall be charged at the rate of ten cents\nper folio for all folios in excess of ten folios for matter in any such\nstatement showing the date, type, amount, and object or purpose, of\nobligations authorized to be issued but which in fact had not been\nissued as of the date of preparation of such statement.\n Any such fee shall be fifty cents less for a certified or exemplified\ncopy of any such debt statement if a previously prepared typewritten or\nphotographic copy shall be presented to the state comptroller for\ncertification or exemplification. The state comptroller shall make no\ncharge for searching for any such debt statement if and when a copy,\ncertified copy, or exemplified copy of the debt statement searched for\nis furnished to the person making such request.\n Except as above provided, the state comptroller shall charge a fee of\none dollar for searching for any debt statement filed in his office\npursuant to the provisions of this chapter.\n
