Rescission form – Effect of modification of forms.

A. The Department of Human Services shall prescribe forms for

the acknowledgment of paternity and the denial of paternity, which

shall be filed with the State Department of Health, Division of

Vital Records, pursuant to Section 1-311.3 of Title 63 of the

Oklahoma Statutes.

B. The rescission of the acknowledgment of paternity shall be

prescribed by the Department of Human Services and made available at

the same locations as the acknowledgment of paternity form provided

for in Section 1-311.3 of Title 63 of the Oklahoma Statutes.

C. A valid acknowledgment of paternity, rescission of

acknowledgment of paternity or denial of paternity is not affected

by a later modification of the prescribed form.
