Form of acknowledgment
Official statutory text
An acknowledgment by individuals of any instrument affecting
real estate shall be in substantially a form as provided for in the
Uniform Law on Notarial Acts or in substantially the following form:

________ personally appeared ________ to me known to be the
identical person(s) who executed the within and foregoing
instrument, and acknowledged to me that ____ executed the same as
