Nonemergency patient form - Perjury

The Insurance Commissioner shall develop, by rule, a form to be
presented to patients by health care providers prior to rendering
nonemergency services. The form shall be designed to seek
information from the patient to further determine the eligibility of
the patient for benefits under the patient’s insurance policy.
Making false statements on the form shall be regarded as willful
misrepresentation.
