address - Surrender on demand.

A. A person holding a license to practice perfusion in this

state may use the title "licensed perfusionist" and the abbreviation

"L.P.".

B. A license holder must:

1. Display the license in an appropriate and public manner; or

2. Maintain on file at all times during which the license

provides services in a health care facility a true and correct copy

of the license in the appropriate records of the facility; and

3. Keep the State Board of Examiners of Perfusionists informed

of any change of address.

C. A licensure issued by the Board is the property of the Board

and shall be surrendered on demand.
