Application for license or permit

A. Applications for a private school license or a private
school solicitor's license shall:
1. Be filed with the Oklahoma Board of Private Vocational
Schools in the manner and upon forms from the Board or substantially
similar to forms implemented by the Board; and
2. Include but not be limited to the following disclosures:
a. the name, mailing address and telephone number of the
Board for the purposes of directing student complaints
to the Board, and
b. documentation of whether the proposed school will
provide only postsecondary education and training or
if a person who has not yet achieved a high school
diploma or its equivalency will be admitted and under
what specific circumstances.
B. Submissions to the Board shall be date-stamped as of the
date of the physical receipt when the office is open for business.
