Form for student to report allergy

The department, in consultation with the department of health, shall develop a standardized form on which a student with an allergy may report the allergy to the school in which the student is enrolled. The department shall make the form available to all LEAs. An LEA shall require each school in the LEA to use the form to maintain a record of any student who has reported having an allergy. Added by 2020 Tenn. Acts, ch. 628, s 1, eff. 3/20/2020.
